Getting a superbill for insurance reimbursement

A superbill is an itemized receipt with the billing and diagnosis codes insurers ask for. If you paid your doula, lactation consultant, or midwife through Bornbir, they can make you one, and you download it as a PDF to send to your insurance company or your HSA/FSA to claim reimbursement.

Asking for a superbill

Superbills are made by your provider, not by Bornbir, and only for an invoice you've paid in full through Bornbir. Only some providers can make one: doulas, lactation consultants, and midwives in the US.

If yours can, your paid invoice's page says "Need a superbill for your insurer or HSA/FSA? Ask [Name]; it shows up here once they make one." Send them a message. They'll need your legal name and date of birth exactly as your insurer has them, and your member ID from your insurance card helps too.

Downloading your superbill

  1. Open the paid invoice. Click the paid line in your conversation ("You paid $500 · ..."), click View payment in your payment confirmation email, or open Account → Payments and choose the payment.
  2. Click Download superbill (PDF).

If you paid from an invoice link without a Bornbir account, open the same link again; the download appears there. Bornbir doesn't send a separate notice when a superbill is ready, so check the invoice page once your provider tells you it's done.

What's on it

  • Your provider's name, practice address, NPI (marked as verified against the federal NPPES registry), and specialty
  • Your legal name, date of birth, and member ID if your provider added it
  • A diagnosis code (ICD-10) and the place of service
  • Each service with its date, billing code, units, and charge
  • The amount you paid by card, a balance due of $0, and your provider's electronic signature

If anything is wrong, such as your name or date of birth, ask your provider to fix it. When they save the change, your download is replaced with the corrected version.

Submitting it to your insurer or HSA/FSA

Every plan handles this differently, so check with your insurer first. In general:

  • Many insurers accept a superbill with an out-of-network claim. Look for a claim form or an upload option in your insurer's member portal or app, and attach the PDF.
  • For an HSA or FSA, submit the superbill to your account administrator as proof of the expense.
  • Keep a copy, and ask your insurer if they need anything else, such as a claim form of their own.

Bornbir doesn't bill insurers or file claims for you, and a superbill isn't a guarantee of payment. Whether you're reimbursed, and how much, depends on your plan.

If the payment is refunded

A superbill says you paid in full, so it can no longer be downloaded once the payment is refunded or your card issuer opens a dispute.
